Ferritin 101: The Difference Between “Iron” and “Stored Iron”

Most people only hear about “iron” in the context of red blood cells, but ferritin tells a different part of the story. Ferritin is the protein that stores iron inside your cells for later use, essentially your body’s iron savings account. Serum iron and hemoglobin reflect what’s circulating right now, while ferritin reflects your reserves.

This distinction matters clinically. Ferritin can drop for months, sometimes years, before hemoglobin or a standard CBC ever flags a problem. By the time hemoglobin is low enough to be labeled “anemia,” the underlying iron depletion has often been building for a long time. This is why so many people feel exhausted, foggy, and unwell while being told their labs are unremarkable; the test that would catch the problem early, ferritin, is frequently left off the standard panel altogether.

It’s also worth noting that ferritin is an acute-phase reactant, meaning it can rise in response to inflammation, even when true iron stores are low. This is one reason ferritin should never be interpreted alone. A thorough evaluation typically pairs ferritin with iron, total iron-binding capacity (TIBC), transferrin saturation, and inflammatory markers like CRP, so the full picture, not just one number, guides the plan.

The Root Causes Behind Low Ferritin and Anemia

Low ferritin rarely has a single cause. In our clinic, we look across several systems at once to understand why your body isn’t holding onto or absorbing enough iron.

1. Poor Gut Absorption and Digestive Dysfunction

Iron is absorbed primarily in the small intestine, and that process depends on adequate stomach acid and a healthy intestinal lining. Low stomach acid, chronic use of acid-blocking medications, increased intestinal permeability (leaky gut), and small intestinal bacterial overgrowth (SIBO) can all interfere with how much iron actually makes it into your bloodstream, no matter how much you’re eating or supplementing.

2. Celiac Disease and Gluten-Related Gut Damage

Unexplained iron-deficiency anemia is one of the most common presenting signs of celiac disease and gluten sensitivity. Ongoing gluten exposure in a sensitive person damages the villi lining the small intestine, the same tissue responsible for absorbing iron, B12, and folate. Anemia that doesn’t respond to supplementation is a classic reason we screen for celiac disease and non-celiac gluten sensitivity.

3. Chronic Inflammation and Autoimmune Activity

Persistent inflammation triggers the release of a hormone called hepcidin, which blocks iron from being absorbed in the gut and locks existing iron away in storage instead of releasing it for use. This pattern, known as anemia of chronic disease, is common in people with inflammatory bowel disease, rheumatoid arthritis, lupus, and other autoimmune conditions. It’s a key reason ferritin can look “normal” or even elevated on paper while true iron availability to your tissues stays low.

4. Thyroid Dysfunction

Thyroid hormone helps regulate stomach acid production and red blood cell formation. When thyroid function is low, or when autoimmune thyroid disease is present even with a normal TSH, iron metabolism often suffers alongside it. We frequently see low ferritin and sluggish thyroid function occurring together, each one making the other harder to resolve until both are addressed.

5. Heavy Menstrual Cycles and Unrecognized Blood Loss

Heavy or prolonged periods, fibroids, and slow gastrointestinal blood loss (from ulcers, hemorrhoids, or inflammatory conditions) are straightforward but frequently overlooked contributors to falling ferritin. Because this blood loss can be gradual, it’s easy to underestimate just how much iron is being lost over time.

6. Diet, Nutrient Cofactors, and Absorption Blockers

Low intake of iron-rich foods, especially in vegetarian and vegan diets, plays a role, but it’s rarely the whole story. Iron absorption also depends on adequate vitamin C, B12, copper, and stomach acid. Frequent coffee or tea with meals, high-calcium supplements taken alongside iron, and certain medications can all blunt how much iron your body actually takes up.

7. Chronic Stress, Poor Sleep, and Blood Sugar Swings

Chronic stress and unstable blood sugar keep the body in a heightened inflammatory state, which, as noted above, can suppress iron absorption through the same hepcidin pathway. Poor sleep further limits the body’s capacity for tissue repair and healthy red blood cell turnover.

What a Standard Iron Panel Often Misses

Many people are told their iron is “fine” after a CBC alone, or after ferritin is checked once without the surrounding context. Questions that are often left unasked include:

  • Is ferritin being interpreted alongside inflammatory markers, or in isolation?
  • Has celiac disease or gluten sensitivity ever been ruled out?
  • Is there an undiagnosed autoimmune or thyroid condition driving chronic inflammation?
  • Is stomach acid adequate, and are digestive symptoms being taken seriously?
  • Is menstrual blood loss, or another source of ongoing blood loss, being accounted for?

Standard care often responds to low iron with a generic supplement and a recheck in a few months. When levels don’t improve, or symptoms persist despite “normal” numbers, that is usually a sign the underlying driver hasn’t been identified yet, not that the anemia is imaginary.
